Adult Day Care & Home Assistive Services
Young at Heart Partners with SOCS (Senior Outing & Care Services)
Young at Heart is proud to announce its sister Non-For-Profit organization that was created to provide financial relief for programs to the seniors in our communities. The State of IL has cut funding to many programs, including crucial senior services. Young at Heart and SOCS work together in order to try a bridge the gap that may exist in preventing those in financial need from getting high quality services they deserve.

SOCS is accepting donations of all kinds in order to redistribute accordingly in our community.
All donations can be made in person to Young at Heart, 6504 Joliet Rd., Countryside, 60525. Checks/money orders can be made out to SOCS and mailed/dropped off to YAH as well - if donating money, please specify which SOCS program you would like your contribution to go towards.
Many participants that are fortunate enough to come to the adult cay facility Young at Heart operates cannot come as often as they would like or need to due to financial restrictions. SOCS is proud to organize an on-going program that collects money put directly towards the daily services provided to participants.
Need a Ride Program
In its beginning stages of planning, this program is being developed in order to help address the shortage of affordable, safe door-to-door transportation for seniors. Once this program is fully developed, it will try to bridge the gaps in our community to provide seamless service to those financially restricted and otherwise homebound.
SOCS is a 501(c)(3) organization and operated exclusively for charitable purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended, or to any corresponding provision of any future federal tax law, as describe above. All donations are tax deductible under the IRS tax code as a public charity. Look us up on the IRS website here.
